Hey guys, the shaft on my lift is spinning inside the pulley. Do I need to try and line up the "holes" and place a nail or pin in there? Should I also loosen the set screws in the second pic and try knocking the pulley back on the shaft more? Can someone confirm? TIA

yes on both accounts, the slot is called a key way and the part that is sheared or fallen out on yours is the key.

Do you think one of these will fit? Not sure what "standard" is.
I do not know your lift. You'll need to measure the width of the slot (since keys are square) and order accordingly. Chances are one of these will fit.
 
Your local hardware store should have an assortment of keys to choose from.
