When I want to really make it from scratch, this is the recipe that I use. It's from a tex-mex restaurant that went out of business (Tejas), but they made it table side so the recipe lives on.

2 whole avocados
2 1/2 oz pico de gallo
1/2 fresh squeezed lime
1/8 tsp Kosher salt
3-6 cloves roasted garlic
10 drops Tabasco green pepper sauce
2 drops Worcestershire sauce
 
In a pinch or feeling lazy this is pretty darn good, most grocery stores around me carry it, even Wal-Mart.

I have always used fresh ingredients. . . . .

Avacados
Diced Cherry Tomatos
Chopped Red Onion
Minced Garlic
Lime Juice
Salt
Canned Sweet Corn
Sometimes I add REAL Crumbled Bacon
Also experimented with mexican cheese blend.

My wife makes an unbelievable dip that we take to the lake most weekends. Some people call it Texas Caviar, but she gets all pissed off when I call it that because it's a little different. I'll try and find the recipe but im pretty sure it has the following in it:

diced avocado
diced red onion
cilantro
corn
black beans
salt
maybe garlic?
small amount of ranch dressing
Rotel tomatoes with green chiles

This stuff is addictive!!!
