ICU update. Starting day 5.
Only one more death in the ICU since my last post. Two transfered to the floor that never got intubated. No one coming off the vent so far. In the last 2 months we have had 7 folks beat this on the vent and come off. Some of those went home finally this week. Glimmers of hope.

Also new study from The Lancet Infectious Diseases (reputable journal). Remdesivir doesn't really work. We're down to monoclonals before you get sick enough and vaccines before you even get sick. And steroids in the hospital. Not much else data driven otherwise

Sorry. Edited my post for some breaking news.

What's the current guidance on monoclonal antibodies? Who and when?

I believer every center is a bit different, and I suspect it boils down to supply of the cocktail. For us you need to have ONE of the high risk criteria.
- Age >64
- Preggo
- DM
- Obesity
- heart disease or hypertension
- sickle cell disease
- some congenital anomalies
- kidney disease
- immunosuppressed.
- Chronic lung disease.

Also needs to be early (less than 10 days) and not that sick (not needing oxygen due to covid)

I have heard of some other places that give it to everyone. I guess they have more?

I know of healthy kids who have received monoclonal antibodies in Arkansas. If you might need them please ask.

No need at this point, but want to be ready if I need it. Does being vaccinated impact the decision? Since I'm vaccinated, in my late fifties and borderline in some of the factors, I was planning to ride any Covid out. I didn't know if it was to the point of being automatic for the antibodies.

Vaccinated folks still can get Regeneron. Remember there's a time limit at some centers. I would recommend you take to your doctor.

Let me put it this way. If I get covid, I'm asking for Regeneron day 1

Thanks, makes sense. Just clarifying, I'm not positive or feeling sick, just getting my facts because it's out there at a pretty high level again.

FDA votes against boosters for age 16 and up. Looks like mainly because they didn't think that young people don't benefit as much from booster. More votes to happen soon.

Why are MC antibodies being throttled? Is there truly a supply shortage?

Where? In our case yes. Supply is lower than demand. So we have to triage. Elsewhere? No idea

@haknslash, monoclonal antibody supply varies by state. I believe they all are purchased by the federal government then dolled out to states who pass them on to treatment centers.

The last estimate I saw was that production was equivalent to 1/6th the rate of new CoVID cases.

It also is important to consider that while vaccines cost $4 to $20 a dose the monoclonal antibodies are thousands of dollars per dose. Someone has to pay for this stuff someday.

In my state of Alabama they’re saying there will be an upcoming temporary shortage. Just trying to understand how the production and supply chain works, lead times etc. Surely hope this is not some stupid stunt by the WH because of our state officials stance on vaccine mandates. I’d love to know how they ration out the supply like which places are deemed x amount vs other states. Is it population driven or what?

@haknslash,

The allocation system I was thinking of ended in the spring. https://www.phe.gov/emergency/events/COVID19/investigation-MCM/cas_imd/Pages/allocation.aspx

Looks like state ordering of doses exceeded production so the feds are taking a new role.

That link says nothing about demand outpacing supply

It smells of an administration pushing back against states that won’t play their immoral ballgame. So instead, they seem to just pull another “Afghanistan maneuver” and willingly let people die I suppose if they don’t do what they say (mandates). MC antibodies has proven to be great to keep folks out of the severe level of this disease and from overloading the hospitals but to this administration that doesn’t seem to be the goal after all. I don’t buy the shortage story especially when half the country is vaccinated and at least 63% are partially vaccinated. IMO Seems like more strong arm tactics while playing with peoples lives.

Saw this yesterday

The drug is called ivermectin, and their message was that it was a stunningly good Covid-19 treatment — “good enough to end the pandemic at any point you wanted,” said Weinstein, an evolutionary biologist and now podcaster associated with the intellectual dark web movement. But they argued that the powers that be are trying to keep word from getting out, preferring instead to push a profitable Big Pharma vaccine.
